Breast Revision with Motiva SmoothSilk® Implants, Galaflex®, and Fat Grafting
This middle-aged woman previously underwent a submuscular (under the muscle) breast augmentation but was unhappy with the results. She experienced pectoralis animation deformity — a condition where the breast implants move unnaturally every time the chest muscles flex. In addition, she developed capsular contracture, a hardening and distortion of the implants due to scar tissue.
Dr. Salvatore Pacella, a board-certified plastic surgeon specializing in complex revision breast surgery, performed a complete implant removal with capsulectomy, eliminating the distorted capsule. To restore a more natural shape and reduce animation, the new implants were placed above the muscle (prepectoral plane) for better aesthetics and improved comfort.

To support the new implant position and prevent future sagging or migration, Dr. Pacella used Galaflex®, an absorbable mesh scaffold, which integrates with the body’s tissue for long-term stability.
Additionally, about 80cc of fat was transferred to the inner breast and cleavage zone—a technique known as lipo-filling—to soften contours, blend implant edges, and create a fuller, more natural-looking décolletage.
