Bilateral Breast Reconstruction Following Radiation with Latissimus Flap & Silicone Implants

This 57 year- old female had a history of bilateral lumpectomies and extensive radiation damage following breast cancer on both breasts many years earlier. Unfortunately, her breast cancer recurred on the right side. She decided to undergo bilateral mastectomies. An immediate reconstruction with a latissimus flaps and tissue expanders was performed on both sides through an incision around the areola. Several months later, the expanders were removed and permanent moderate-profile silicone implants were placed. In addition, a nipple reconstruction and fat transfer were also performed. Postoperative result at 1 year shows excellent symmetry with minimal scarring, and a significant improvement in radiation damage.

Breast Reconstruction Patient 6803

Bilateral Immediate Breast Reconstruction with Tissue Expanders Following Nipple-Sparing Mastectomy

This 28 year-old female was diagnosed with left-sided breast cancer. Because of her young age and increased lifetime risk, she opted for bilateral mastectomies. The removal of her breast was performed through small incisions on the side of the breasts. The nipple tissue was preserved and a two-stage breast reconstruction was performed with MENTOR® ARTOURA™ Breast Tissue Expanders. Several months later, the expanders were replaced with the most advanced, new “gummy-bear” MemoryShape™ silicone implants. Fat transfer to the upper portion of the breast was also performed to create a very natural contour. Postoperative result at 6 months shows excellent symmetry and shape, with a patient’s increase in breast volume as she requested.

Breast Reconstruction Patient 6141

Bilateral Immediate Breast Reconstruction with Latissimus Flap & Silicone Implant Following Skin-Sparing Mastectomy

This 40 year-old female developed right sided breast cancer. Because of her young age and increased lifetime risk, she opted for bilateral mastectomies. Skin-sparing mastectomies were performed through a circular incision around the areola. An immediate breast reconstruction was performed with bilateral Latissimus muscle flaps and tissue expanders. After several months, the expanders were replaced with moderate-profile silicone gel implants. A nipple reconstruction was also performed with local skin flaps and skin grafts from the groin area. The resulting “scarless” breast reconstruction is pictured at 2 years postoperatively. The aesthetic breast shape continues to be ideal years after the reconstruction.

Bilateral Immediate Breast Reconstruction with Latissimus Flap & Silicone Implant Following Areolar-Sparing Mastectomy

This 40 year-old female developed left sided breast cancer. Because of an extensive family history of breast cancer, she opted for bilateral mastectomies. Bilateral areolar-sparing mastectomies (which removes the breast and nipple but leaves behind the natural transition of the areola) were performed by her breast surgeon. At the same time, bilateral immediate breast and nipple reconstructions with a latissimus muscle flap (from the upper back) and a tissue expanders were performed. The skin on the latissimus muscle was repositioned to create a new nipple. Several months following expansion, the expanders were removed and changed to a moderate-profile silicone gel implants. Postoperative result at 1 year shows natural-appearing, symmetric breasts with excellent projection.

Bilateral Immediate Breast Reconstruction with Tissue Expanders Following Nipple-Sparing Mastectomy

This 41 year-old female had an extensive family history of breast cancer and tested positive for the BRCA gene. Because of her high risk of developing breast cancer, she opted for bilateral prophylactic mastectomies. The removal of her breasts were performed through small incisions around the bottom of the nipple. Immediate breast reconstruction was performed with MENTOR® ARTOURA™ Breast Tissue Expanders. Several months later, the expanders were replaced with the most advanced, new “gummy-bear” MemoryShape™ silicone implants. Fat transfer to the upper portion of the breast was also performed to create a very natural contour. Postoperative result at 1 year shows excellent symmetry and shape, preservation of the nipple tissue, all designed to the patient’s desire to also increase breast volume.

Right Sided Breast Reconstruction with Latissimus Flap & Silicone Implant Following Areolar-Sparing Mastectomy

This 47 year-old female developed right sided breast cancer and was recommended by her oncologist to undergo a right sided mastectomy. A right sided areolar-sparing mastectomy (which removes the breast and nipple but leaves behind the natural transition of the pigmented areolar skin) was performed by her breast surgeon. At the same time, an immediate breast and nipple reconstruction with a latissimus muscle flap (from the upper back) and a tissue expander was performed. The skin on the latissimus muscle was repositioned to create a new nipple. Several months following expansion, the right-sided expander was removed and changed to a high-profile silicone gel implant. In the left breast, a lower profile silicone implant was placed for symmetry. Postoperative result shows a natural-appearing and symmetric right breast reconstruction at 1 year.

Bilateral Immediate Breast Reconstruction with Latissimus Flap & Silicone Implant Following Skin-Sparing Mastectomy

This 51 year-old woman developed an aggressive right sided breast cancer which extended to the skin surface above the right nipple. In addition, she had underlying breast asymmetry with her left breast being smaller than right since a young age. Bilateral mastectomies were performed. She required a large resection of skin above the right nipple to remove the tumor which resulted in a vertical scar above the right nipple. An immediate breast reconstruction was performed with bilateral Latissimus muscle flaps and tissue expanders. Several months later, the expanders were replaced with slightly different size moderate-profile silicone gel breast implants (i.e. larger implant on the left to account for underlying asymmetry). In addition, a nipple reconstruction was performed with local skin flaps and skin grafts from the lower abdomen. Postoperative result at 2 years shows excellent symmetry and a natural breast contour.
